PRESENTATION TO GP's

THE OUTLINE

 

•  Aim : To show you how and who to recommend in full confidence to which therapy & in the way to best help the patient with ES and prevent him becoming the problem patient

•  We will do all this in full realization of how pressed you are for time and instead of adding a burden we will relieve of you by preventing another Problem Patient: the ultimate drain on your resources.

•  The basis of all treatment success is grounded on the Doctor Patient relationship.

•  In the case of ES this is threatened by the inability of the Patient to readily accept the GP's recommendations. This leads to dissatisfaction & increase utilisation of facilities.

•  The GP does not belief that it is possible in his clinic to offer non-medication interventions even though they are recognised as being the most effective intervention in certain kinds of ES

•  This is wrong:

•  It is possible to give the non-medication interventions BUT NOT BY THE GP HIMSELF in the form of self help

•  These interventions, which do work extremely effectively, when given to the right patient, will have a greater benefit if the GP can honestly recommend them .

•  Let us understand the basic mechanism, its pathology and possible treatment strategies in ES [MODEL]

•  Let us understand the ALGORITHM which helps chose the correct treatment

•  Let us examine briefly the treatment protocols:

•  Medications:

• i. Is mood disturbance the leading symptom: Is he mainly sad or anxious?

• ii. Is thought the leading symptom: Is he Obsessive or Ruminating?

•  Non-Medications:

• i. Mood: Relaxations, Time Management, Exercise

• ii. Thoughts: Learning 4 trends about how he relates in general to himself and others & in stress provoking situations how he relates to himself and others.

•  How to explain to the patient and enlist him:

•  Non confrontational

•  Seeking consent and agreement for each stage

•  Using "presenting symptom" as stressor and not product of stress

•  Stress now increases symptom & vicious cycle.

•  Re-iterating Model to enlist and explain therapy to Patient [ enforcing it with GP]

•  Referring to chosen therapy
